Legislators are much more likely to remember a personal story than a form letter.  Consider adding a couple of sentences to share:

• Which medical equipment/supplies you use
• How access to these products and services help you stay healthy at home
• How your local medical equipment supplier has helped you or your family
• What would happen if you couldn’t get your equipment/supplies when you need them
