Improve Community Health and Eliminate Health Disparities

Urge Congress to Remove Barriers to Health

Improving the health of individuals and communities is at the core of Trinity Health’s Mission. This includes a commitment to those who are poor and vulnerable. One way to accomplish this is by addressing social influencers of health (SIOH) in our communities across the nation.

But we can’t do this work alone. When we ask Congress to focus its attention on impacting key issues affecting our populations related to housing, access to food, maternal care, and health disparities, we can have a greater influence on those who depend on us for their care.

As policymakers consider their priorities this year, your elected leaders are interested in understanding what you have to say about important issues that impact the communities they serve.

Now is the time to bring these issues to the attention of Congress and ask your leaders to engage with their communities directly. Please take a few minutes to urge Congress to learn more and work to connect social and clinical care, improve health access and outcomes, and eliminate health disparities for all.
