The One Big Beautiful Bill Act (H.R. 1) continues to move through Congress, paired with a larger budget reconciliation package and a proposed $9.4 billion rescissions measure (H.R. 4). While these legislative efforts aim to reduce federal spending, they currently offer no savings through Medicare Advantage and no relief for Medicare physicians.
As it stands, the package does not include a Medicare physician payment fix for 2025. The -2.83% cut to physician reimbursement remains in place, with no adjustments anticipated until 2026. This marks the fifth consecutive year of payment reductions, with Medicare physician payments down by 33% since 2001 when adjusted for inflation.
However, separate legislation has been introduced that could address this pressing issue. Sen. Roger Marshall (R-KS) recently introduced S. 1640, the Medicare Patient Access and Practice Stabilization Act of 2025, the Senate companion bill to H.R. 879. This bill proposes an 8.51% increase in Medicare physician reimbursement, reversing this year’s cut and adding an inflation-based update. While not currently part of the reconciliation package, these bills represent a potential path forward to stabilize physician practices and preserve access to care.
